Pelvic Pain

Fibroids and Endometriosis

Symptoms

A 34 year old woman presented for a second opinion. She had extreme dysmenorrhoea.

Diagnosis

An earlier laparoscopy had not alleviated her symptoms. At Laparoscopy an 8cm fibroid at the fundus of the uterus was noted.

Procedure

This was removed by dividing the attachment between the fibroid and the uterus.

Outcome

There was minimal endometriosis at the back of the uterus (Pouch of Douglas) which was removed. The pain resolved post operatively.

The fibroid (right) is attached to the uterus on a small base

The base of the fibroid is tied off

The normal uterus after the fibroid is removed

Endometriosis behind the uterus (Pouch of Douglas)

The endometriosis has been removed from the Pouch of Douglas
